目的：评价桑蒺合剂干预高血压病（肝火亢盛夹痰证）患者外周血单核细胞表面Toll样受体4（TLR4）表达的作用。方法：60例高血压肝火亢盛夹痰证患者，随机分为治疗组和对照组各30例，均给予非洛地平或联合贝那普利治疗，治疗组加用桑蒺合剂，观察周期28天，检测治疗前后血压、中医证候积分、外周血单核细胞表面TLR4表达。结果：治疗后两组患者血压水平、中医证候积分、TLR4表达均较治疗前降低，其中治疗组中医证候积分、TLR4表达较对照组改变更明显（P<0.05）。结论：桑蒺合剂可改善肝火亢盛夹痰证患者的临床症状，减少外周血单核细胞表面TLR4表达，在一定程度上可以抑制免疫反应。

This study was aimed to observe the effect of Sang-Ji (SJ) mixture in the treatment of Toll-like receptor 4 (TLR-4) expression on peripheral blood monocytes (PBMCs) in hypertensive patients. A total of 60 cases of hypertensive cases with overabundant liver-fire and phlegm syndrome were randomly divided into the treatment group and the control group with 30 cases in each group. Both groups received Felodipine and/or Benazapril treatment. The treatment group was added with SJ mixture. The observation duration was 28 days. The detection was made on indicators such as blood pressure, traditional Chinese medicine (TCM) syndrome scores and TLR-4 expression on PBMCs before and after treatment. The results showed that after treatment, the blood pressure, TCM syndrome scores and TLR-4 expression on PBMCs of both groups decreased compared with that of the pretreatment. Changes on TCM syndrome scores and TLR-4 expression of the treatment group were more significant than that of the control group (P < 0.05). It was concluded that SJ mixture improved clinical symptoms in patients with overabundant liver-fire and phlegm syndrome, decreased TLR-4 expression on PBMCs, in order to inhibit the immune response to a certain extent.
