目的:探讨基于极速成像技术的原发性高血压患者的脉象客观化研究。方法:对67例原发性高血压患者及63例健康人(对照组)行颈动脉极速成像技术检测及桡动脉脉象检测。应用法国Supersonic Aixplorer彩色多普勒超声诊断仪,内置UltraFast极速成像技术检测颈动脉脉搏波传导速度(pulse wave velocity,PWV),包括收缩期起始(beginning of the systole,BS)时及收缩期结束(ending of the systole,ES)时的PWV;应用DDMX-100脉象仪检测桡动脉脉象参数,包括h1、h3、h4、h5、h3/h1、h4/h1、h5/h1、w1、w2、w1/t、w2/t;分析两组数据,并采用Spearman非参数法对两种技术参数进行相关性分析。结果:原发性高血压组h1、h3、h3/h1、h4/h1、w2/t增高,h5/h1降低(P < 0.05)。BS和ES时,高血压组的PWV均显著高于对照组(P < 0.01)。对照组BS、ES时的PWV与h3/h1、h4/h1呈正相关(P < 0.05),原发性高血压组BS、ES时的PWV与h4、h3/h1、h4/h1呈正相关(P < 0.05)。结论:原发性高血压患者脉象参数与极速成像技术参数间具有良好的相关性,进一步完善了脉象的客观化研究。

[Abstract]
Objective: To objectively study the pulse manifestation of patients with essential hypertension based on UltraFast imaging technique. Methods: A total of 67 cases of patients with essential hypertension and 63 healthy persons(control group) were chosen in this research. All cases were examined by synchronously applying the UltraFast imagingtechnique and pulsopraphy. The color Doppler ultrasonography (French Supersonic Aixplorer) was used, and the built-inUltraFast imaging was used to detect the pulse wave velocity (PWV) of carotid artery, including the PWV of the beginning of the systole (BS) and the ending of the systole (ES). The parameters of pulse presentations of radial artery were detectedby application of DDMX-100 pulsography, which including h1, h3, h4, h5, h3/h1, h4/h1, h5/h1, w1, w2, w1/t, w2/t. Allthe parameters were researched and compared between the two groups, and all the parameters were also compared bySpearman non parametric method to analyze the correlation between the parameters of the two kinds of techniques.Results: Compared with the control group, h1, h3, h3/h1, h4/h1, w2/t of pulse manifestation parameters were increased inthe essential hypertension group and h5/h1 decreased (P < 0.05). At BS and ES, the PWV of the essential hypertensiongroup was significantly higher than those of the control group (P < 0.01). The PWV of BS and ES of the control groupwere positively correlated with h3/h1, h4/h1 (P < 0.05), while those of the essential hypertension group were positivelycorrelated with h4, h3/h1, h4/h1. Conclusion: The parameters of pulse presentations in patients with essentialhypertension have significant correlations between pulse parameters and UltraFast imaging technique parameters, andfurther improve the objective research of pulse condition.
