目的 通过观察电针“四关”穴及其相应单穴对脑卒中后抑郁(post-stroke depression,PSD)大鼠行为学及海马神经脑源性神经营养因子(Brain-derived Neurotrophic Factor,BDNF)、蛋白激酶类家族 B(Tropomyosin-related kinase B,TrkB)蛋白表达变化,研究电针四关穴治疗PSD时的抗抑郁机制,并探索四关穴的治疗中两穴是否具有协同作用。方法 将40只SD大鼠随机分为空白组、模型组、四关组、合谷组及太冲组。除空白组外,余大鼠均予PSD造模,空白组及模型组不做针刺,余组均电针相应穴位治疗,连续3周。通过多次糖水消耗实验及敞箱实验观察大鼠的抑郁行为学变化,采用免疫组化SP法检测海马BDNF及TrkB含量。结果 ①PSD造模后,各组大鼠糖水消耗比、穿行格子数、竖起修饰次数等行为学均较空白组明显下降(P < 0.05);②治疗后各组大鼠敞箱实验穿行格子数,四关组高于模型组(P < 0.05);合谷组在21天高于模型组(P < 0.05);太冲组在14天高于模型组(P < 0.05),第21天与模型组比无显著差异(P > 0.05)。③治疗后敞箱实验竖起修饰次数和糖水消耗比变化均为:四关及太冲组均高于模型组(P < 0.05),合谷组与模型组比无明显差异(P > 0.05)。④与模型组比较,四关组、太冲组及合谷组BDNF、TrkB阳性细胞数均增高(P < 0.01),合谷组BDNF、TrkB阳性细胞数少于四关组(P < 0.05)。结论 电针四关能显著改善PSD大鼠行为学指标,在提升敞箱实验穿行格子数时,合谷穴及太冲穴起到协同作用,在提升敞箱实验竖起修饰次数和糖水消耗比时,太冲穴起到主要作用;电针四关改善PSD大鼠行为学指标的作用机制可能与提高BDNF、TrkB蛋白表达,进而改善神经元生存、发育及功能有关;四关穴治疗PSD时的协同作用得到部分体现,但仍需进一步研究。

Objective To explore the antidepressant mechanism of electroacupuncture at “Four Gate” point and its corresponding acupoint on the behavior of post-stroke depression (PSD) rats and the expression of Brain-derived Neurotrophic Factor (BDNF) and Tropomyosin-related kinase B (TrkB) in hippocampus. BDNF and Tropomyosin-related kinase B (TrkB), to investigate the antidepressant mechanism of electroacupuncture at the “Four Gate” point in the treatment of PSD, and to discuss whether the two points have synergistic effects in the treatment process.Methods Totally 40 SD rats were randomly divided into blank group, model group, "Four Gate" point group, Hegu (LI4) group and Taichong (LR3) group. All the other rats were subjected to PSD modeling excepted for the normal group, “Four Gate” point group, Hegu group and Taichong were electroacupunctured at corresponding points each day, while blank group and model group were not treated for 3 weeks. The behavioral changes of depression in rats were observed by repeated sugar consumption test and open box test, and the contents of BDNF and TrkB in hippocampus were detected by immunohistochemistry SP method.Results ①After PSD modeling, compared with the blank group, the consumption ratio of sugar and water, the number of traversing grids and the number of vertical modifications of rats in each group decreased significantly (P < 0.05); ②After treatment, the number of lattice in the open box test of rats in each group was significantly higher than that in the model group (P < 0.05); the Hegu group was higher than the model group from 21 days (P < 0.05); Taichong group was higher than the model group on day14 (P < 0.05), but on day 21, there was no significant difference between the two groups. ③ After treatment, the change trend of the times of vertical modification and the ratio of sugar to water consumption in the open box experiment were roughly synchronous. The changes of “Four Gate” point group and Taichong group were significantly higher than those of model group (P < 0.05), and there was no significant difference between Hegu group and model group (P > 0.05). ⑤ Compared with the model group, the number of BDNF and TrkB positive cells in Siguan group, Taichong group and Hegu group were significantly increased (P < 0.01), while the number of BDNF and TrkB positive cells in Hegu group was less than that in Siguan group (P < 0.05), and there was no significant difference between Taichong group and Siguan group (P > 0.05).Conclusion Electroacupuncture at “Four Gate” point can significantly increase the behavioral indexes of rats. Hegu and Taichong play a synergistic role in increasing the number of lattice through the open box experiment. Taichong plays a major role in increasing the number of modification times and sugar water consumption ratio in open box experiment. The mechanism of electroacupuncture at “Four Gate” point in improving behavioral parameters in PSD rats related to increasing the expression of BDNF and TrkB protein probably, which in turn improves neuronal survival, development and function. the synergistic effect of “Four Gate” point on PSD is partly reflected but further research is needed.
