[摘要]
目的 观察“百会、神庭、水沟”穴电针联合跑轮训练对大脑中动脉闭塞(middle cerebral artery occlusion, MCAO)脑缺血再灌注(cerebral ischemiac reperfusion, CIR)损伤模型大鼠神经及运动功能重建的影响。方法 SPF级雄性SD大鼠50只,取10只体重相近的设为假手术组,将另40只采用Longa线栓法制作CIR模型并选取其中造模成功的32只采用随机数字表法均分模型组、电针组、跑轮组和电针+跑轮组,每组8只。电针组选“百会、神庭、水沟”穴电针治疗;跑轮组进行自主跑轮训练;电针+跑轮组治疗同电针组和跑轮组;治疗5次/周,共4周。采用经皮层电刺激记录运动诱发电位(motor evoked potential, MEP)并结合Zea-Longa神经学评分评价神经传导功能;采用悬吊、Grip test评分及腓肠肌张力实验评估肢体肌力和躯体平衡及运动功能的协调性;大鼠血脑屏障通透性采用伊文思蓝渗透法观察。结果 电针组Zea-Longa评分及MEP各时相点潜伏期低于跑轮组,MEP各时相点波幅峰值高于跑轮组(P<0.05)。跑轮组Grip test评分、悬吊时间和等长收缩肌力均高于电针组,MEP各时相点波幅峰值低于电针组(P<0.05)。而电针+跑轮组Grip test评分、悬吊时间、MEP各时相点波幅峰值及等长收缩肌力均高于电针组和跑轮组,Zea-Longa评分及MEP各时相点潜伏期均低于电针组和跑轮组(P<0.05)。结论 电针有利于神经传导功能重建,跑轮训练可促进CIR模型大鼠骨骼肌重塑和运动功能恢复,在防治脑缺血再灌注损伤时二者具有协同作用。

Objective To observe the effects of electroacupuncture at "Baihui, Shenting, Shuigou" points combined with running wheel training on the nerve and movement of rats with middle cerebral artery occlusion (MCAO) cerebral ischemiac reperfusion injury (CIR) The impact of functional reconstruction.Methods There were 50 SPF male SD rats. 10 rats with similar weight were set as the sham operation group. The other 40 rats were made CIR model by longa thread bolt method, and 32 of them were selected. They were equally divided into Model group, electroacupuncture group, running wheel group and electroacupuncture + running wheel group by random number table method, with 8 rats in each group. The electroacupuncture group was treated with electroacupuncture at Baihui, shenting and Shuigou points; The running wheel group received independent running wheel training; The treatment of electroacupuncture + running wheel group was the same as that of electroacupuncture group and running wheel group; Treatment 5 times / W for 4 weeks. Transcortical electrical stimulation was used to record motor evoked potential (MEP) and combined with Zea-Longa neurological score to evaluate nerve conduction function; Suspension, grip test score and gastrocnemius tension test were used to evaluate the coordination of limb muscle strength, body balance and motor function; The permeability of blood-brain barrier in rats was observed by Evans blue permeation method.Results The Zea-Longa score and the latency of MEP in electroacupuncture group were lower than those in running wheel group, and the peak amplitude of MEP was higher than that in running wheel group (P<0.05). The grip test score, suspension time and isometric systolic muscle strength in the running wheel group were higher than those in the electroacupuncture group, and the peak amplitude of MEP was lower than that in the electroacupuncture group (P<0.05). The grip test score, suspension time, peak amplitude of MEP and isometric systolic muscle strength in electroacupuncture + running wheel group were higher than those in electroacupuncture group and running wheel group, and the Zea-Longa score and latency of MEP were lower than those in electroacupuncture group and running wheel group (P<0.05).Conclusion Electroacupuncture is beneficial to the reconstruction of nerve conduction function. Running wheel training can promote skeletal muscle remodeling and motor function recovery in CIR model rats. The two have a synergistic effect in preventing and treating cerebral ischemia-reperfusion injury.
